Available Programs
For over thirty years, Family Alliance has served adults aged 50 and better throughout McHenry County and the surrounding area, who are experiencing mental, physical, emotional, or social challenges.
We are home to THREE distinct programs:
- The REACH Wellness program is an active adult wellness program provided 5 days per week and designed to meet the needs of persons who require more structure, may be socially isolated and at risk for depression, or in need of some physical assistance.
- The REACH Memory Care program is similar to the REACH Wellness program, but for persons experiencing memory loss associated with Alzheimer's disease or other forms of dementia. The program includes an on-site day program (offered 5 days per week), in-home assessments, and care partner education.
- The Mental Health Recovery program provides mental health services that includes individual therapy and counseling, psycho-social rehab groups, community support, and case management both on site and in the individual's home.
In addition to our program options, we provide on-site access to services that includes geriatric psychiatry, chiropractic medicine, and massage therapy. To best address the needs of those we serve, Family Alliance is committed to continued program growth.
Family Alliance services are provided by a highly-trained multidisciplinary team that includes Registered Nurses, Licensed Clinical Social Workers, Certified Nursing Assistants, Activity Specialists, and Community Support Specialists.
We are accredited by the Joint Commission and the Commission on Accreditation of Rehabilitation Facilities. In 2011, Family Alliance received the National Adult Day Center Award from NADSA (National Adult Day Services Association).
Free Support Groups
At the Family Alliance Facility:
- Alzheimer/Dementia Caregiver Support Group
- This group is for anyone giving care to someone affected by Alzheimer's disease or other forms of dementia. The group meets on the 3rd Friday of each month, 1:00 pm - 2:30 pm.
- Spousal Caregiver Support Group
- For spouses giving care to someone affected by Alzheimer's disease of other forms of dementia. The group meets on the 2nd Monday of each month, 10:30 am - 12:00 pm. Contact: Inez Young at (815) 385-4672.
- Bereavement Support Group
- If you are struggling to find peace after losing a family member or friend, then this support group could be for you. A Licensed Clinical Social Worker will be available to facilitate the group and offer educational information on grief and loss. This group is offered on the 2nd and 4th Monday of each month from 6:30 pm - 8:00 pm. Contact Nicole O'Dea at (815) 333-8626.
At Sunrise Assisted Living in Crystal Lake
- Alzheimer/Dementia Caregiver Support Group
- For anyone giving care to someone affected by Alzheimer's disease or other forms of dementia. The group meets at Sunrise Assisted Living of Crystal Lake on the 3rd Wednesday of each month, starts at 5:30 pm. RSVP to (815) 444-6600 by the 2nd Wednesday of each month.
At Del Webb- Sun City Huntley
- Alzheimer/Dementia Caregiver Support Groups
- For residents of Sun City who are giving care to someone affected by Alzheimer's disease or other forms of dementia. The groups meet at Sun City Huntley in Fountain View "F" at these times: 4th Monday of each month, 10:30 am - 12:00 pm.
