Community Support

Family Alliance’s community support staff provides support, assistance and advocacy to seniors who are in need of financial, legal, medical and social services.

Our Community Support Staff:

• Assess, identify and prioritize a senior’s basic needs and implements a plan of action to fulfill those needs.
• Provide one-on-one intervention to ensure that their client receives the medical, rehabilitative, social and legal services as determined by the initial assessment.
• Provide on-going advocacy and solution-based assistance for the senior.
• Provide outreach and follow-up depending on the client’s physical, social and mental situations.
