Care Management

Our clinical professionals provide short-term and long-term eldercare assessments to benefit the senior and their caregivers. Family Alliance’s team of care management professionals can clarify and alleviate health care anxieties, develop senior care options and assist in acquiring appropriate health resources.

The Care Management Program includes:

• Assessment and Evaluation of the senior - Our clinical staff will identify health, personal, cognitive, financial and social support needs of the senior.
• In-Office Consultation - With key family members, available options and resources will be identified to meet their elderly family member’s needs.
• Care Plan - Services that are most appropriate for the senior and their caregiver’s abilities will be determined. These services may include: in-home care, counseling, day health care, respite, financial resources and/or caregiver support.
