Dementia
Dementia is part of a number of different illnesses, Alzheimer’s Disease being the most common of those conditions. Dealing with dementia is difficult for caregivers as well as for the senior with the illness. The Dementia Services provides:
- In-home assessment
An Registered Nurse, certified in geriatric specialties (RN,C) will come to your home to evaluate the needs of the senior with memory loss, answer your questions about dementia, link you to available resources and develop a plan that will consider the needs of your entire family.
- Education
An RN,C and Dementia Counselor will help you to understand dementia and the many possible symptoms. Both professionals are available to help you, as a caregiver, learn how to take care of your own needs, too.
- Support
We can help you establish a strong base of support from community agencies, relatives, friends, and Family Alliance’s counseling services and support groups.
- Follow-up
Our RN,C will continue to monitor your case, keeping you connected with the services and people you need. Remember – NO ONE SAYS YOU HAVE TO PROVIDE ALL THE CARE ALONE..
